Therapy sessions
When clients book for therapy I take their name, date of birth, address, contact number and GP information for safeguarding reasons. This can help identify a person if they go missing or if there is a concern for their safety or the safety of someone else. I also take their email address as a secondary method of contact if the client chooses to use this, which is stored electronically on a password-protected computer.
During clients’ sessions I will take handwritten notes, which are stored in a lockable box in my office. The office is locked when not in use and this is also where the clients’ paper records including personal data are stored securely.
Personal data is stored until 7 years after the client has completed therapy with me, at which point paper records are securely shredded and electronic records are deleted.
Personal data will never be shared with a third party unless the client is at risk, is suffering harm or is a risk to others.
By booking an appointment with me, you consent to the above.
